Lay Training Committee Member Form

You will need 3-7 members.

Please include the names,  phone numbers, and email addresses for the members of your Lay Training Committee.

**Please indicate which of them has agreed to be the convener of that group.** If you have any questions about how this form, your LTC members, or any other issues, please contact the Office of Field Education and Supervised Ministry before filling out the form!!

Your LTC convenor will need to attend the LTC orientation (virtual meeting) on August 20, 2022.
